⑴ Official in charge of disbursement of the procuring entity : Yoshida Kiyoshi, Executive Vice President In Charge of Finance

⑶ Nature and quantity of the products to be rent : Hemodialysis system 1 Set
⑷ Rent period : From 1 April, 2023 through 31 March, 2028
⑸ Rent place : Kobe University Hospital
⑹ Qualifications for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall :
A not come under Article 3 and 4 of the Regulation concerning the Contract for Kobe University Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause,
B have the Grade A, Grade B or Grade C qualification during fiscal 2022 in the Kinki area in offer of services for participating in tenders by Single qualification for every ministry and agency, or in tenders by Kobe University,
C prove to have the ability to deliverthe products, required by this notice, on the date and to the place specified by The Obligating Officer,
D prove to have the ability to rent the products concerned by themselves and by a third party, should the products requested through this notice be rent by a third party, prove to have prepared a system to provide maintenance for the rent products,
E prove to have obtained license for leasing a medical instrument in accordance with the Pharmaceutical Affairs Law,
F not be currently under a suspension of business order as instructed by Executive Vice President In Charge of Finance.
