1. The Philippine Health Insurance Corporation Regional Office X, through the Corporate Operating Budget of CY 2021 intends to apply the sum of Five Million Four Hundred Five Thousand One Peso and Eighty Four Centavos
(P5,405,001.84) being the ABC for the first year of payment and Sixteen Million Two Hundred Fifteen Thousand Five Pesos and Fifty Two Centavos (P16,215,005.52) for three years being the total ABC to payments under the contract for the Procurement of 1 Lot Multi-Year Contract for Janitorial Services for PhilHealth Regional Office-X, LHIOs and its PBCs. Bids received in excess of the ABC shall be automatically rejected at bid opening.

2. The Philippine Health Insurance Corporation Regional Office X now invites bids for the above Procurement Project. Delivery of the services is required by June 1,2021 to May 31, 2024. Bidders should have completed, within the immediately preceding three (3) years from the date of submission and receipt of bids, a contract
similar to the Project. The deion of an eligible bidder is contained in the Bidding Documents, particularly, in Section II (Instructions to Bidders).

3. Bidding will be conducted through open competitive bidding procedures using a nondiscretionary "pass/fail" criterion as specified in the 2016 revised Implementing Rules and Regulations (IRR) of Republic Act (RA) No. 9184.

a. Bidding is restricted to Filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines, and to citizens or organizations of a country the laws or regulations of which grant similar rights or privileges to Filipino citizens, pursuant to RA No. 5183.

4. Prospective Bidders may obtain further information from PhilHealth Regional Office X and inspect the Bidding Documents at the address given below during office hours from 8:00 a.m. to 5:00 p.m. However, due to alternative work arrangement being adopted, please set an appointment first with the BAC Secretarial through the cellphone number given below, subject to the limitations provided by the No-Contact Rule.

5. A complete set of Bidding Documents may be acquired by interested Bidders on April 27-May 17, 2021 from 8:00 a.m. to 3:00 p.m. (Mondays thru Thursdays), from 8:00 a.m. to 11:00 a.m. on Fridays, except for May 17, the deadline will be on 8:00 a.m. only: from the given address and website(s) below and upon payment of the
applicable fee for the Bidding Documents, pursuant to the latest Guidelines issued by the GPPB, in the non-refundable amount of PhP25,000.00. The Procuring Entity shall allow the bidder to present its proof of payment for the fees in person.

6. The PhilHealth Regional Office X will hold a Pre-Bid Conference on May 5, 2021, 10:00 a.m. at a venue to be announced later, which shall be open to prospective bidders. Only one (1) representative per bidder shall be allowed to attend in consideration of health and safety protocols.

7. Bids must be duly received by the BAC Secretarial through manual submission at the office address indicated below on or before May 17,2021,9:00 a.m. Late bids shall not be accepted.

8. All Bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB Clause 14.

9. Bid opening shall be on May 17, 2021, 10:00 a.m. at a venue to be announced later. Bids will be opened in the presence of the bidders' representatives who choose to attend the activity. Only one (1) representative per bidder shall be allowed to attend in consideration of health and safety protocols.

10. For manual submission: The bidder shall submit one (1) sealed mother envelope, containing 1 original set of documents and 3 certified true copies, as specified in the Instruction to Bidder Clause 20 and Markings of the envelopes

11. The Philippine Health Insurance Corporation Regional Office X reserves the right to reject any and all bids, declare a failure of bidding, or not award the contract at any time prior to contract award in accordance with the revised IRR of RA No. 9184, without thereby incurring any liability to the affected bidder or bidders.
