⑴ Official in charge of disbursement of the procuring entity : Takuyuki Ise, Director, National Hospital Organization (NHO) Ishikawa National Hospital
⑵ Classification of the services to be procured : 22, 31
⑶ Nature and quantity of the services to be required : respirator 1 set
⑷ Rental period : From August 1, 2022 to July 31, 2023
⑸ Rental place : NHO Ishikawa National Hospital
⑹ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall :
① not come under Article 5 of the Regulation concerning Contract for NHO. Furthermore, minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under casesof special reasons within the said clause ;
② not come under Article 6 of the Regulation concerning Contract for NHO ;
③ have Grade A, B or C in "Service Offer" for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in Tokai-Hokuriku Region ;
④ prove to have prepared a system to provide rapid response for maintenance of the procured service ;
⑤ meet the qualification requirements which the Obligating Officer may specify in accordance with Article 4 of the Regulation ;
⑺ Time-limit for tender : 5 : 00 P.M. July 19, 2022
